An example of a pre-screener form used by a disconnected and paper based recruiting center

This was sent to a database member but they still needed to ask for name, phone and age. All things they have in their database.

Why did they ask then?

Because this survey is not connected to their database so when the results come back they need to know who sent it. Then it is printed out so someone can call and leave a message.  It will circulate around the call center possibly to be found, but likely not, when the member calls back.
